Many programs focus on preventing readmissions and providing case management for those with the greatest risk of hospitalization. However, this is where a comprehensive prevention program significantly reduces hospitalizations and prevents disease for beneficiaries with both the greatest risk and those with rising risk starts. A long term, patient-centred approach using well-trained preventive clinicians helps patients change behaviour and reduce their risk.
