Simple Feedback Survey
The technique has been borrowed from Philip Rosedale‘s interview with Kevin Rose on the Foundation Video Series (YouTube) in the interview Philip mentioned a technique he always did as CEO to gauge feedback and to gauge a trend line, on a very frequent basis he would send a survey out to all the employees asking them three simple questions:
- One a scale of 1–5 (1 being really bad and 5 being really good) how well am I doing at my job right now?
- What was the key motivation for giving me that rating?
- Is there anything I can improve on?

Creating the Survey
You can use any free or paid survey tool as the feature set you will need is very basic. Below is a basic step-by-step for creating the survey:
- Create a new survey, ideally name it “QX — 20XX”
- Create a basic instruction text saying something like “This survey is purely about [Persons Name]l, the role he plays within the organisation and your overall feedback. Please answer the survey honestly as all feedback is anonymous.”
- Next, create a rating module with a 1–5 scoring and give it the question “How well am I doing at my job right now?”
- Next, create an open text box (long text) and name the question “What was the key motivation for giving me that rating?”
- The Last step is then to create another open text box (long text) with the question being “
- Is there anything I can improve on?”
Recommendations
That’s pretty much it, you have complete the survey, I would recommend the following:
- You should send an email with a link to the survey to all the people you have worked a lot with over the past quarter, half year or more
- Create a nice email about wanting to gauge feedback for self improvement
- Send the email with a deadline of about 2 weeks
- At the end of the first week, send a nice little poke reminder email
- On the Last day send out another poke email
- Do not try to identify who said what, but grasp the bigger message on how well you’re doing and what are the areas mostly highlighted for improvement
