Care for vulnerable patients is often fragmented with little coordination between primary care, specialists, behavioral health, and social services. This leaves many patients and their families not only feeling frustrated, but suffering worse health outcomes as a result. Additionally, chronic illnesses (disproportionately prevalent in older adults) have risen significantly in recent years due demographic shifts brought on by aging Baby Boomers, among other factors. While leading-edge Baby Boomers have already aged into the Medicare program, this demographic momentum will continue to accelerate over the coming years.

The number of practitioners equipped to serve this growing user base is not expected to keep up with demand. It is already a major problem.

This is where ConcertoCare is stepping in, and stepping up.

As a value-based care provider for seniors and other adults, ConcertoCare is making healthcare work for the patients who need it the most, and in the place where they are often best served: in their homes. ConcertoCare enhances its patients’ autonomy, health, and well-being through a patient-centered, tech-enabled approach to at-home care. Their three-part strategy — encompassing CarePartners (a wraparound service), Geriatric Primary Care (their DCE offering), and Program of All-Inclusive Care for the Elderly (PACE) — is creating incentive alignment to reduce acute care needs by enhancing preventative interventions in an effort to bend the cost curve.

  1. CarePartners: Designed for patients with complex care needs and existing primary care providers, ConcertoCare’s CarePartners program allows patients to keep their existing doctors and benefit plans while leveraging ConcertoCare’s expansive suite of holistic health offerings. Through virtual and in-person conversations and collaboration with a patient and their existing PCP, CarePartners delivers additional at-home services to help maintain a patient’s physical and emotional health, address any isolation they may be experiencing, and enhance their overall independence.
  2. Geriatric Primary Care: ConcertoCare’s DCE offering provides primary care house calls and orchestrates a range of services to help patients maintain their physical and emotional health and independence at home. ConcertoCare’s primary care clinicians, nurses and social work care managers learn what matters most to patients and then bring together an orchestra of pharmacists, behavioral health clinicians, social workers, health coaches, and home health aides customized to a given member’s care needs. Additional at-home services are provided, using both in-person and virtual interactions, allowing patients to maintain their physical and emotional health, address isolation, and support their independence.
  3. PACE: Expanding upon legacy PACE offerings for members who wish to remain independent and in their homes but whose medical conditions meet the criteria for nursing home-level care, ConcertoCare’s upleveled approach decentralizes the traditional center based model, empowering members to live their lives in the environments in which they feel most themselves. ConcertoCare’s interdisciplinary care team orchestrates the right combination of home-based services, complemented by a range of services and social activities including adult day care, meals, occupational therapy, and social work counseling, among other offerings.

ConcertoCare’s continued expansion in these three verticals is supported by the ongoing shift towards a Medicare Advantage-aligned future that is well underway. In 2020, 39% of all Medicare beneficiaries — 24M overall — were enrolled in outcomes-oriented Medicare Advantage plans, an adoption curve that has steadily increased over time since the early 2000s. Between 2019 and 2020 alone, total Medicare Advantage enrollment grew by about 2.1 million beneficiaries, or 9%, underscoring demand for more comprehensive and holistic solutions. This market momentum shows no indication of slowing down.
