The current policy landscape for SDOH-related reforms constitutes a sprawling ecosystem of incentives, penalties, measurements, and investments across over a dozen programs. This complexity is driven by the heterogeneity of the market, in which any given health system is participating in a unique combination of federal, state, and county or city-level programs, some of which are mandatory and others voluntary, all of which are influenced by national and local politics, regional demographic and economic trends.

What follows is a high-level summary roundup of key federal programs and initiatives that are helping bring new approaches to addressing SDOH into health systems and local markets around the country. Most are recent programs with dedicated reimbursement for enhanced coordination services, non-clinical wraparounds, or specific interventions to address certain determinants such as housing or transportation.
