Information about Care Coordination Organizations and People First Care Coordination

As you know, Medicaid Service Coordination (MSC) is transitioning to Care Coordination Organizations/Health Homes (CCO/HH).

New York State’s Office of People with Developmental Disabilities has prepared an online resource center that provides information regarding People First Care Coordination and the creation of Care Coordination Organizations (CCOs).

Among the various resources available on OPWDD’s website are information videos about People First Care Coordination and a video about CCOs, which are also provided below.

Advance Care Alliance of New York (ACANY), the Care Coordination Organization/Health Home (CCO/HH) QSAC is affiliated with, has been initially designated as a Care Coordination Organization/Health Home (CCO/HH) by the New York State Department of Health and the Office of People with Developmental Disabilities. The transition of Medicaid Service Coordination (MSC) to Care Coordination is still planned for July 1, 2018 pending state and federal approval.
