Currently, SuperPhone® tracks how many people add themselves to your contacts via each form. In order to allow you to always keep track as well, you may only ENABLE or DISABLE a form. To do this, visit this link while logged in https://app.superphone.io/settings/forms
Your default form is your main form and the one you should customize:
All other forms can be disabled or renamed for future use:
