New document requested before arrival

Pre-fill before arrival for Test & Go, Sandbox Programme and AQ.
Regarding to the latest announcement from Thailand’s Government upon entry scheme has been updated.
All arrival passenger must provide required document on RT-PCR for 2 times and The Order of Communicable Disease Control, Officer at Port Health and Quarantine.
Please print out and fill in the information as attached below for Test & Go,Sandbox programme and AQ.
