Il pulsiossimetro è utile per la diagnosi di polso paradosso in pediatria (e a tutte le età)
Classicamente il polso paradosso è descritto nel tamponamento cardiaco, ma anche nell’asma e nel croup è un segno di gravità, per cui anche in presenza di una saturazione di ossigeno non allarmante trovare un polso paradosso è una indicazione al monitoraggio continuo.
Si parla di polso paradosso quando la pressione arteriosa sistolica si abbassa durante l’inspirazione più di 10 mmHg.
Inspiration is performed by decreasing intrathoracic pressure. Because the heart is in the thorax, this pressure is transmitted to the heart and causes a bigger gradient between the pressure in the veins outside of the thorax and the right atrium/ventricle. As such, with each inhalation, more blood flows into the right side of the heart. When this occurs, the interventricular septum subtly bulges into the LV cavity, lowering LV filling, and therefore lowering systemic stroke volume/BP. Because the ventricle can normally also expand outward, this septal shift is usually small, and the difference in the blood pressure is therefore small between inspiration and expiration (<10 mmHg). In states in which the ventricle cannot expand outward (e.g. tamponade) or in which the drop in intrathoracic pressure with inspiration is profound (e.g. status asthmaticus), the septal shift is exaggerated and the difference in BP is larger.(2)
Si tratta di una misurazione difficile da ottenere nel bambino: si veda la modalità classica con cui deve essere registrato nella segnalazione della Stanford University (2). Evidenziabile però con facilità con un pulsiossimetro in grado di visualizzare anche la curva pletismografica (1)
Croup - Pulsus paradoxus
"As intrathoracic pressure becomes more negative, afterload on the left ventricle increases and cardiac output decreases. Since the pulse oximeter waveform amplitude is proportional to arterial blood volume, there is a decrease in amplitude during inspiration known as pulsus paradoxus. The magnitude of depression during inspiration has been consistently correlated with the severity of extrathoracic airway obstruction and can be used to monitor disease progression or resolution" (1)Asthma — Pulsus paradoxus
“The elevated airways resistance raises intrathoracic pressure and, much as in croup, causes a transient decrease in cardiac output during inspiration. The consequential pulsus paradoxus correlates with the severity of the asthma exacerbation” (1)
