1*PTW is completed of the required certificiad, attachment and skecthed.

2*After all hse documentation has been finished ptw cordinator side should be verify this document if there is no mistake, hse cordinator will aprove.

3*Nextly decumentation procedure has been finished, authorization side wiil perform job according to instructions the required by ptw.

4*Then all safety working finished, each side just performing and operating authority should be go on working activity then they inspected working area.

5*According to inspeciton success, they will sign this part.

6*After working completed, working area must be clear then operating authoirty side has been checked.

7*Finally they are sign this document next ptw files goes to archive.

WHO IS OPERATING AUTHORITY:

Operating authority: Ensure a good standard of housekeeping is maintained during the work and that all equipment is removed upon completion;

Operating Authority

Shall check the PTW mitigation measures, inspect the work place and ensure that all the precaution stated on the PTW are in place before work commence.

Declare that the working area has been checked and accepted once the work is completed.
