The Permit to work (PTW) system is a formal recorded process used to control works which are identified as potentially hazardous.

The Permit is a detailed document which authorizes certain people to carry out specific work at a specific site at a certain time, and which sets out the main precautions needed to complete the job safely.

The permit-to-work is a documented procedure that authorizes certain people to carry out specific work within a specified time frame.

It sets out the precautions required to complete the work safely, based on a risk assessment.

Who is the responsible person on the permit when an accident happened?

a)Performing Authority (who work done on side)

Face shield, knee protection okey but others .. Lack of safety. Safety glass, without fire blanket work, grinding machine does not has machine guard etc..

b)Operating Authority (who inspected on side)

Lack of safetry inspection while working at height activity !

c)HSE Department

Safety team consists of many parts (fire and rescue team, supervisor, ptw coordinator etc.) We dont like to see rescue team because if you see them we are in definitely big trouble

d)All signatures are responsible which situated on the permit.

Before sign at documents, think twice and inspect site !

Your signature on a PTW imposes certain legal duties on you, including your personal accountability in case of an accident (if you did not fulfill adequately your duties)!
