Typically a need for PTW is identified from:

• Project Risk Assessment and
• Site specific PTW procedures, which will give a guidance on types of jobs (relevant for this site) requiring PTW.

Additionally, it can be considered whenever it is intended to carry out any work which may adversely affect the safety of personnel, the environment or the plant.

First we prefer apply permit to work systems for most dangerous proess (enter the tank, working at high platform, heavy lifting) but it will not enough for huge project. Because of if you want to managed successfully more than 100 subcontractor you need to alway follow up what are you doing on site.
